What financial document is needed for this step?

You need to provide your 2024 Federal Tax Return (specifically the Adjusted Gross Income – AGI).

What if I don’t have that document?

If your 2024 Federal Tax Return is unavailable or your income has significantly decreased due to certain life changes, you can provide alternative documentation, such as:

• W-2s

• Unemployment compensation statements

• 1099s or bank statements showing earnings

• Social Security, pension, or annuity statements

• Military retirement benefits

• Employer statements on company letterhead

If your tax return is unavailable, you must explain why and submit the Household Summary Form with your supporting documents. This form will be available at enrollment.
